Please provide us also with the official registered address(es) of this (these) persons and the coordinates of the director(s) (if any). Please provide us also with the passport data of all the physical persons concerned, a biography of them, data about their education and about their current job and place of work.

3. Have you ever been active in any medical activity? Do you own or/and manage any medical center? If yes, please provide us with its (their) name, location, address, list of services provided, list of working specialists, date of foundation, a precise description of any licences used, certified copies of these licences and their appendixes, size of the premises and number of patients in the database.
